Enrolling at the Health and Counselling Centre

The benefits for enrolled patients include:

Who can enrol?
Any MIT student or staff can choose to enrol, if eligible for publicly funded healthcare.*

If you are transferring from another practice, the form you sign when enrolling at our centre gives permission for the retrieval and transfer of your notes from your previous doctor.

The enrolment form is available from the Health and Counselling Centre, South Campus, S Block, Gate 9, Otara Rd (under Library building).

Students under 16 years of age must have parental consent to enrol as a patient at the Health and Counselling Centre so must fill out a different patient enrolment form. This form is available from the Health and Counselling Centre.

Do I have to enrol to access services?
No. It is your choice. As an MIT student or staff you can still access Health and Counselling services as a casual patient.

Can I go to other medical centres and still remain your patient?
Yes you can, but you will be expected to pay full casual patient fees at the other medical centre as you can only be enrolled at one medical centre. You do not have to sign an enrolment form in order to be seen elsewhere.

If you do sign another enrolment form elsewhere you will be removed from our practice’s enrolment register and your patient records will be transferred to that medical centre.

* Non NZ residents are not eligible for public funded healthcare, however some exceptions apply.

What happens to enrolment information?
Personal health information is protected under strict privacy laws. When you enrol, the PHO will use the information collected from you to build an enrolment register which is securely sent to the Ministry of Health (MOH) where your information will remain confidential. This information includes your name, address, ethnicity, and the dates on which you see the doctor or nurse.

Patient Charges
A full list of standard patient charges is displayed in the reception area of the Health and Counselling Centre.  If you have any questions, please ask our staff.

Community Services Card (CSC)
Students who are 16 years old and over and are NZ citizens or permanent residents may qualify for a Community Services card which can make access to some health care services cheaper.

Application forms are available at the Health and Counselling Centre or you can download an application form from the Work and Income website or phone Work and Income NZ toll free on 0800 999 999.

Payment of fees
All consultations are to be paid on the day of your appointment. Our Health and Counselling Centre accepts cash, cheques and has an Eftpos facility. Please let us know if there is any difficulty in paying your account on the day as we do pursue unpaid accounts.
