Privacy
We work within the guidelines of the Health Information Privacy Code 1994 in accordance with the Privacy Act 1993, to ensure your health information is protected. In order to ensure optimum care we may need to provide your relevant details and medical information to other health providers who are involved in your care.
We must have your consent when transferring any identifiable health information about you to someone who is not involved in your care e.g. insurers. When you sign an enrolment form you will sign consent for us to retrieve your patient notes from your previous doctor. We are required to securely send limited
demographic information for funding and administrative purposes which remain confidential.
Complaints
If you are unhappy with our service you can let us know or make a formal complaint. The process for this is available at the centre or by contacting:
